How can I manage this with biztalk.
Let's consider I've >
doctor 1 - patient A
doctor 1 - patient B
doctor 1 - patient C
doctor 2 - patient U
doctor 2 - patient Z
How can I have in my report :
doctor 1 - patient A, patient B, Patient C
doctor 2 - patient U, patient Z

Is your question about mapping or the complete Integration process?
If its regarding mapping then
Use Looping functoid pointing the Source Doctor node to destination node Doctor.
Map other fields as usual under doctor node.
The map will loop through all Doctors and for each Doctor node it will map the child fields.
